Estate information form

This form gathers what I need to apply for probate and settle an estate: details about the person who died, their family, their will, and what they owned and owed.

Fill in what you can, then drop it off at the office or scan it and email it to chris@taskey.ca. My assistant, Kelsey, will follow up about anything missing. “I don’t know” is a fine answer for now.

Two ways to fill it in

What the form doesn’t ask for

I use what you send to reply to you, to check for conflicts of interest, to understand your matter and, if you become a client, to do the work. Sending the form doesn’t make you a client on its own. Questions about your information: Christopher Taskey, chris@taskey.ca. Privacy
